Please complete the following agency nurse application form as accurately as possible and a member of our recruitment team will be in contact with you shortly. To complete you application and to comply with Irish Healthcare legislation each agency nurse must supply the following documents before being approved to work. Upon receipt of this application a member of our team will be in contact with you in relation to these documents.

Your completed online application must be accompanied with the following documents and certificates.

• Copy of current An Bord Altranais Registration
• 2 passport photographs
• Copy of passport / driver's license
• Copy of current CPR certificate (completed within the last 2 years)
• Copy of current Manual Handling Certificate (completed within the last 2 years)
• Occupational Health Records:

    - BCG Records (if applicable in country of origin)
    - Evidence of BCG scar or a validated mantoux test & result
    - Hep B Immunisation Record - Anti HBS levels
    - Immunisation Records to include Diptheria, Tetanus & Polio
    - Rubella and Varicella Antibodies (validate samples)
    - Measles and Mumps Antibodies (validate samples)
    and for high exposure risk:
    - Hep B Surface Antibodies
    - Hep B Surface Antigen
    - Hep C Antibody

• Copy of valid working visa
• GNIB card (if applicable)

The Garda Vetting form will be sent to you upon your application being processed succesfully.
