Senior Social Worker – Palliative Care Service, Co Louth

Senior Social Worker required for Specialist Palliative Care Services, Co. Louth.

The Social Worker will be responsible for providing a medical social work services for clients of the specialist palliative care service. This will include assessment, crisis intervention, counselling, family work, discharge planning with other team members, and helping patients and families access health, social and welfare entitlements in the community.

You will work closely with other team members in responding to the social and emotional impact of terminal illness and hospitalisation on patients and their families. This will extend to bereavement follow up where appropriate.

We are interested in speaking with social workers with experience in the following areas:

·      Counselling

·      Patient & family support

·      Psychological assessment

·      Discharge planning

·      Linking in with external agencies

·      Knowledge of working with the Nursing Home Support / Fair Deal Scheme

The Candidate:

·      A recognised qualification as a Social Worker and registration with CORU

·      A minimum of at least 3 years post graduate experience in a medical setting or working with a multi-disciplinary team will be considered.
